Authorization Release
United Way of Forsyth County reserves the right to take photos and video on United Way of Forsyth County property, at United Way hosted events, or at general events open to the public.
By registering for and/or participating in United Way of Forsyth County events,
• I hereby authorize and consent that United Way of Forsyth County shall have the absolute right to copyright, publish, use and/or assign any and all photographic portraits or pictures, video and/or sound recordings, or any part thereof, they have taken or made of me in which I may be included in whole or in part, whether apart from or in connection with, illustrative or written printed matter, story or news item, motion picture, television or radio spots, or for publicity, advertising, or any other lawful purpose whatsoever, in conjunction with my own or a fictitious name, or in reproductions thereof in color or otherwise.
• I hereby waive all claims for any compensation for such use or for damages.
• I hereby waive any right that I may have to inspect and/or approve the finished product or the advertising copy that may be used in connection therewith or the use to which it may be applied.
• I hereby warrant that I am of full age and have every right to contract in my own name in the above regard.

Privacy Policy
Our Commitment to Our Donors
We will not sell, share or trade our donors’ names or personal information with any other entity, nor send mailings to our donors on behalf of other organizations. This policy applies to all information received by United Way of Forsyth County (UWFC), both online and offline, as well as any electronic, written, or oral communications.

- Collection of Personal Information from Users. Except as specifically set forth herein, UWFC may collect or track: (a) email addresses of users (b) information knowingly provided by the user in online forms; (c) aggregate and user-specific information regarding which pages a user accesses; (d) the home server domain names, date and time of your visit. UWFC may use Google Analytics or a similar service to gather, store, and track certain information related to your visit to and activity on the Sites. When a user performs a search within the Sites, UWFC may record information identifying the user or linking the user to the search performed and recording information related to the search request.
- Use of Personal Information Collected. Personal information collected by UWFC may be used by UWFC for many reasons, for example, for recordkeeping, editorial and feedback purposes, marketing and promotional purposes, statistical analysis of users’ behavior, product development, content improvement, or to customize the content and layout of the Sites. Individually identifying information, such as names, postal and email addresses, phone numbers, and other personal information which users voluntarily provide to UWFC may be added to UWFC’s databases and used for future mailings regarding new products or services and upcoming events.
- Disclosure of Personal Information to Third Parties. Individually identifying personal information provided voluntarily by users will not be sold or otherwise disclosed to third parties not affiliated with UWFC. Identifiable and non-identifiable user information and data may be disclosed or distributed to another party with which UWFC enters or reasonably may enter into a corporate transaction, such as, for example, a merger, consolidation, acquisition, or asset purchase, or to a third party pursuant to a subpoena, court order, or other forms of legal process, in response to a request by or on behalf of any local, state, federal, or other government agency, department, or body, whether or not pursuant to a subpoena, court order, or other forms of legal process, or if determined by UWFC in its sole judgment that such disclosure or distribution is appropriate to protect the life, health, or property of UWFC or any other person or entity. Identifiable and non-identifiable user information and data that users provide to UWFC may also be provided to third parties that contract with UWFC to provide products, information or other services to UWFC. While UWFC may undertake efforts to see that any third party to whom UWFC discloses personal information is under an obligation to use the personal information solely for the purposes for which the information was disclosed, such third parties are independent third parties over which UWFC exercises no control. UWFC is not responsible for, and will not be liable for, the conduct, actions, omissions, or information handling or dissemination practices of third parties.
- The website may also contain links to third party websites. The UWFC website may contain links to other internet web sites for the convenience of users in locating information, products, or services that may be of interest. If you access a third-party website from a link on the UWFC website, any information you disclose on that website is not subject to this Privacy Policy. It is possible that these links may be used by third parties or others to collect personal or other information about you. UWFC is not responsible for the privacy practices of such websites, advertisers, or third parties or the content of such websites and it is the user’s sole obligation to review and understand the privacy practices and policies of these other websites. UWFC does not control these third parties’ use of cookies, collection of information, or how they manage such information. It is solely the user’s obligation to review and understand the privacy practices and policies of these other websites, advertisers, and any third parties.
- Internet Security.UWFC uses reasonable efforts to ensure that the website is available. However, there will be occasions when access to the website will be interrupted or unavailable. UWFC will use reasonable efforts to minimize such disruption where it is within its reasonable control. You agree that UWFC shall not be liable to you for any modification, suspension or discontinuance of the website. You understand that the technical processing and transmission of any content via the website may be transferred unencrypted and involve: (a) transmissions over various networks; and (b) changes to conform and adapt to technical requirements of connecting networks or devices. Please be advised that we do not guarantee that any information sent from our website will be secure during transmission, nor can we guarantee the confidentiality of any communication or material transmitted to UWFC via the website or the Internet in general, including, for example, personal information such as your name or address.
- Comments, Notices and User Content. All materials or comments posted to any aspect of the website are public. As such, UWFC assumes no liability for the accuracy of such information, no duty to update or correct such information, and no liability for such communications arising under the laws of copyright, libel, privacy, obscenity, or otherwise.
- Opt-Out Right.Except as necessary for UWFC to provide the services, information, or products requested by a registered user, by updating the profile preferences on the website, a registered user may opt out of having his or her personally identifiable information, which has been voluntarily provided to UWFC through an account registration, or electronic news or information subscription, prospectively retained by UWFC, used by UWFC for secondary purposes, or disclosed by UWFC to third parties. (This does not apply to collection of the type of Web browser of the user to the Sites, or to non-identifiable user information provided or collected.)
- Access to and Ability to Correct Personal Information.Upon your request via postal mail, email, or telephone, UWFC will provide you with a summary of any personal information retained by UWFC that has been voluntarily provided by you to UWFC through online forms, registration forms, surveys, email, or another online avenue. You may modify, correct, change or update your personal information by contacting UWFC by email at webmaster@unitedway.org.
- Right to Change Privacy Policy.UWFC reserves the right at any time to modify, alter or update this Privacy Policy. Your use of the Sites following any changes means that you agree to follow and be bound by the Privacy Policy as changed. Any change to this Privacy Policy shall be effective as to any user who has visited the Sites before the change was made. It is the obligation of users visiting the Sites before the change to learn of changes to the Privacy Policy since their last visit.

Code of Ethics/Policies
At the core, to care for one another, we have to trust one another.
As volunteers, employees, and representatives of United Way of Forsyth County, we have a responsibility to uphold the highest of ethical standards—both for ourselves and for the benefit of our community. We believe these standards go beyond compliance with laws and regulations—calling us to fulfill higher obligations as stewards of the public trust.
This Code of Ethics is an important way for us to reaffirm our ethical commitments. It sets forth the principles and standards that guide our decisions and actions. As you are probably aware, many of the ideas that are included within the Code have come from discussions with our employees and volunteers over the last few years. This collaborative process has been essential in developing the Code and has made it a document that reflects and is more responsive to our needs.
As we read the Code, we must remember that it is more than a set of standards and responsibilities. It is a way of connecting our values and our ideals with the work we do every day. The Code is not just about what we should do, but why we do it.
Of course, no document can give us all the answers to our ethics questions. This Code is meant to help us further the discussion of ethics in our workplace. It also encourages us to seek guidance and clarification when we have questions about applying our principles and standards. The involvement of all our employees and volunteers is essential to upholding these principles and standards. Only by continuing to work together on these issues can we ensure that the United Way of Forsyth County fulfills its commitment to build and maintain the public trust in all that we do.
- PERSONAL AND PROFESSIONAL INTEGRITY
A personal commitment to integrity in all circumstances benefits each individual as well as the organization. We therefore:
- Strive to meet the highest standards of performance, quality, service and achievement in working towards the UWFC mission.
- Communicate honestly and openly and avoid misrepresentation.
- Promote a working environment where honesty, open communication and minority opinions are valued.
- Exhibit respect and fairness toward all those with whom we come into contact.
- ACCOUNTABILITY
UWFC is responsible to its stakeholders, which include member UWFC organizations, donors and others who have placed faith in UWFC. To uphold this trust we:
- Promote good stewardship of UWFC resources, including donations, grants and other contributions that are used to pay operating expenses, salaries, and employee benefits.
- Refrain from using organizational resources for non-UWFC purposes.
- Observe and comply with all laws and regulations affecting UWFC.
3. SOLICITATIONS AND VOLUNTARY GIVING
The most responsive contributors are those who have the opportunity to become informed and involved. We therefore:
- Promote voluntary giving in dealing with donors and vendors.
- Adhere to our United Way’s noncoercion policy
- Refrain from any use of coercion in fundraising activities, including predicating professional advancement on response to solicitations.

5. CONFLICTS OF INTEREST
To avoid any conflict of interest or the appearance of a conflict of interest which could tarnish the reputation of UWFC as well as undermine the public’s trust in all United Way organizations, UWFC staff and representatives including family members:
- Avoid any activity or outside interest which conflicts or appears to conflict with the best interest of UWFC, including involvement with a current or potential UWFC vendor, grantee, or competing organization unless disclosed to and not deemed to be inappropriate.
- Ensure that outside employment and other activities do not adversely affect the performance of their UWFC duties or the achievement of UWFC’s mission.
- Ensure that travel, entertainment and related expenses are incurred on a basis consistent with the mission of UWFC and not for personal gain or interests.
- Decline any gift, gratuity or favor in the performance of UWFC duties except for promotional items of nominal value, and any food, transportation, lodging or entertainment unless directly related to UWFC business.
- Refrain from influencing the selection of staff, consultants or vendors who are relatives or personal friends or affiliated with, employ, or employed by a person with whom they have a relationship that adversely affects the appearance of impartiality.
UWFC VOLUNTEERS
- Should not knowingly take any action, or make any statement, intended to influence the conduct of UWFC in such a way to confer any financial benefit on themselves, their immediate family members or any organization in which they or their immediate family members have a significant interest as stakeholders, directors or officers.
- Certify that they take no actions to fraudulently coerce, manipulate or mislead the auditors for the purpose of rendering the financial statements materially misleading.
- Disclose all known conflicts or potential conflicts of interest in any matter before the Board of Directors, if they are Board members, or any committee upon which they serve. Limit participation in discussions related to subjects where there may be a potential conflict to providing basic information. Abstain from the voting process in connection with such matters.
- Members of the Board shall annually file with the United Way office a disclosure of all known potential conflicts of interest.
SENIOR FINANCIAL OFFICERS:
- Should provide full fair, accurate, timely and understandable disclosures in periodic reports
- Ensure UWFC’s compliance with applicable governmental rules and regulations.
6. CONFIDENTIALITY AND PRIVACY
Confidentiality is a hallmark of professionalism. We therefore:
- Ensure that all information, which is confidential, privileged or nonpublic, is not disclosed inappropriately.
- Respect the privacy rights of all individuals in the performance of their UWFC duties.
7. POLITICAL CONTRIBUTIONS
UWFC encourages individual participation in civic affairs. However as a charitable organization, UWFC may not make contributions to any candidate for public office or political committee and may not intervene in any political campaign on behalf of or in opposition to any candidate for public office. We therefore:
- Refrain from making any contributions to any candidate for public office or political committee on behalf of UWFC.
- Refrain from making any contributions to any candidate for public office or political committee in a manner that may create the appearance that the contribution is on behalf of UWFC.
- Refrain from using any organizational financial resources, facilities or personnel to endorse or oppose a candidate for public office
- Clearly communicate that we are not acting on behalf of the organization, if identified as an official of UWFC, while engaging in political activities in an individual capacity.
- Refrain from engaging in political activities in a manner that that may create the appearance that such activity is by or on behalf of UWFC.
8. GUIDANCE AND DISCLOSURE
Volunteers, staff, and representatives are encouraged to seek guidance from the Board Ethics Committee (comprised of three members appointed by the Board Chair) and Staff Ethics Officer (appointed by the President) concerning the interpretation or application of this Code of Ethics. Any known or possible breaches of the Code of Ethics should be disclosed. Staff and representatives should contact the President. Volunteers should contact the Chair of the Board Ethics Committee. Reports of possible breaches will be handled in the following manner:
- All reports of possible breaches will be treated in confidence as much as the organization’s duty to investigate and the law allow. If confidentiality cannot be maintained, the individual disclosing the possible breach will be notified.
- All reported breaches will be investigated and, if needed, appropriate action taken based upon the policies of the organization. Retaliation against a person who suspects and reports a breach in good faith will be treated as an independent breach of the Code. UWFC affirms prompt and fair resolution of all reported breaches.
GLOSSARY
Candidate for Public Office: An individual who offers herself or himself or is proposed by others as a contestant for an elected public office, whether such office is federal, state or local.
Contribution, Political: Anything of value, including monetary and in-kind gifts, provided for the purpose of influencing the outcome of an election.
Donors: All individuals and entities that make charitable or in-kind contributions to UWFC.
Immediate Family Members: An individual’s spouse, children, parents, siblings, and spouses of children and siblings.
Nonpublic Information: Any business, financial, or personal information, which is not publicly known or available.
Political Committee: Any party, committee, association, fund or other organization organized and operated primarily for accepting contributions to influence the selection, nomination, or election of any individual to any federal, state or local office.
Privileged Information: Information protected from involuntary disclosure by legally recognized privileges such as attorney-client, doctor-patient, and others.
Promotional Items of Nominal Value: Gifts used to promote an organization’s name, products, or services which have as retail value of $25 or less.
Representatives: Individuals who provide personal services to UWFC as independent contractors, consultants or loaned executives.
Staff: All individuals, who provide services to UWFC as employees or leased employees.
Vendors: Entities which provide goods and services to UWFC for a fee.
Volunteers: All members of the UWFC Board of Directors and committees appointed by the Board of Directors, who perform their UWFC duties without compensation.
